How To Create A Tuition Agreement Form

This guide walks you through how to create a Tuition Agreement Form using Onespot Forms. This form allows schools to collect parentsโ€™ tuition payment preferences (monthly, semesterly, yearly) per child, so administrators can later invoice accurately and consistently.

Important: This form does not charge parents directly. It captures preferences that admins later use to set up invoicing.


When to Use a Tuition Agreement Form

Use this form when:

  • Opening enrollment for a new school year
  • Collecting tuition plan preferences before invoicing
  • Transitioning from another billing system
  • Supporting multiple tuition programs or payment cadences

Step-by-Step: Creating the Tuition Agreement Form

Step 1: Create a New Form Screen

  1. In the app, click the โšก Lightning Bolt.
  2. Go to Content โ†’ New Screen.
  3. Select Make a Form.
  4. Name the screen: Tuition Agreement.
  5. Click Add Content.

(This will open the Onespot Form builder.)


Step 2: Add an Intro / Instruction Section

Before adding questions, include a short instruction section so parents understand the purpose of the form.

How to add the instruction text:

  1. Click the โž• (plus) button in the form builder.
  2. Select Text.
  3. Paste or type your instruction content.
  4. Customize the font style as needed (header, paragraph, bold, alignment, font size).
  5. Save the text block by clicking the โœ“ check button on the right.

Example text:

This form helps us confirm your preferred tuition payment plan for the upcoming school year. Your responses will be used to set up invoicing for each enrolled child.


Step 3: Add Question Blocks & Prefill Parent and Child Information (Recommended)

Before you can use AutoFill or Prefill, you must add a Question block to the form.

How to do this:

  1. Click the โž• (plus) button.
  2. Select Question.
  3. Choose the appropriate question type (for example, Short Answer).
  4. Enter the question text (such as Parent Email or Child Name).

Once the Question block is added, you can then:

  • Click Show Advanced Options on the question
  • Access the Pre-fill Options section
  • Enable AutoFill / Prefill
  • Map the question to existing parent or child information

Using Question blocks with AutoFill helps reduce errors and prevents parents from re-entering information the system already knows.


Step 4: Add Tuition Plan Selection (Per Child)

For each child, add a Question block for selecting a tuition plan.

How to add a tuition plan question:

  1. Click the โž• (plus) button.
  2. Select Question.
  3. Choose Dropdown as the question type.
  4. Enter the question text (for example: Select your childโ€™s tuition program).
  5. Mark the question as Required if needed.

You can add images (such as tuition rate tables or screenshots) to help parents better understand their options.

How to add an image:

  1. Click the โž• (plus) button.
  2. Select Image.
  3. Choose how to add image: Upload, Onespot AI, From URL
  4. Position the image above or below the related question.


Step 5: Add Payment Cadence Selection

For this section, you may use any Item block that best fits how you want parents to select their payment cadence.

Common choices include:

  • Question (Dropdown or Multiple Choice)
  • Text (for instructions or explanations)
  • Product (if you prefer to visually group payment options)

Most schools use a Question block so parents can clearly select one option.

Typical payment cadence options:

  • Monthly
  • Semesterly
  • Yearly

If applicable, you may also clarify variations such as:

  • 9-month monthly plan
  • 12-month monthly plan

Choose the item type that feels clearest for your schoolโ€™s workflow and parent experience.


Step 6: Support Multiple Children

Many families have more than one enrolled child.

Best practice:

  • Repeat the Child + Plan + Payment Cadence section for Child 2, Child 3, and Child 4.
  • Mark additional child sections as optional.

Although uncommon, some families may choose different payment cadences per child, so allow flexibility.


Step 7: Optional Notes or Acknowledgment

You may include:

  • An optional notes field for special circumstances
  • A short acknowledgment such as:

    I understand this form is used to set up my tuition invoices.


Step 8: Review and Publish

Before finishing the form:

  • Click the ๐Ÿ‘ Eye icon to preview how the form will appear to parents
  • Review question order, wording, required fields, and images

When youโ€™re ready to save:

  1. Click the โœ“ Check icon to save your form
  2. Youโ€™ll be prompted to create the Screen/Form
  3. Confirm to finish setup and make the form available in the app

Once completed, the Tuition Agreement form is ready to share with parents.
